Reminder Form

This form allows you to register reminder dates, dates that you would like an email reminder about sending flowers to someone. We send it out several days in advance.

When you have filled out the form to your satisfaction, click on the SEND button at the bottom of the form to register your reminder.

If your date is a fixed, personal one (such as a birthday), enter it here. You can SEND multiple copies of this form if you have more than one date.
